When do you first remember feeling anxious?
Does your anxiety show up occasionally or is it with you most of the time?
Is your anxiety mild, moderate, or severe?
Are there certain types of events or situations that make your anxiety worse?
Do you feel moods like:
Nervous
On edge
Panicky
Always on and you can't shut it off
Do you notice yourself thinking things like:
Overestimating danger when it shows up.
Underestimating your ability to cope with the danger.
Underestimating your ability to obtain help and support.
Worries and catastrophic thoughts.
Do you experience things like:
Sweaty palms
Tight muscles
Rapid Heartbeat
Dizziness
Shallow and rapid breathing
Do you do things like:
Avoiding situations that make your anxiety worse.
Leaving situations when anxiety begins.
Trying to be perfect or to control everything.
Doing things to feel safe and noticing your anxiety get worse when you can't do these things.
